Safer Systems - Saving Lives

A national collaborative initiated by the Australian Council for Safety and Quality in Health Care with the aim to engage hospitals throughout Australia in a commitment to implement six proven interventions to improve patient care and prevent avoidable deaths within the hospital setting.  Southern Health is implementing this initiative across Monash Medical Centre and Dandenong Hospital.

Quality Indicators
Evidence review to provide a summary of the research and evidence on the use of clinical indicators internationally. In particular, to identify hospital wide clinical indicators in use which are most effective as a measure of patient safety and which have the potential to be modified for the Australian context.

NBS Enteral Feeding Guidelines
Development of evidence-based guideline and clinical path for management of oral feeding in preterm infants to decrease length of stay and reduce variability in care in Special Care Nurseries.

Health for Kids
A HARP funded project aimed to improve health outcomes for children and young people in the Southern Health catchment area by supporting evidence-based practice and facilitating partnerships within and between acute and community health services.  For further information see the Health for Kids  website.

NICS Fellowship - From Evidence to Practice: The how and why of implementation
Extensive evaluation of the processes of evidence-based change in a tertiary hospital.

EBHC for Librarians

The aim of this project is to encourage health librarians in self-directed learning through reading, completing exercises and participating in a discussion group on the topic of evidence based practice. By working through the modules, librarians will able to gain an overview of the many aspects of evidence based health care relating to their role as either a librarian or information manager.
