Coordinating Care for KidsThe aim of the project is to improve health outcomes for children and young people who have a disability, a chronic illness or may have been born prematurely and require care from multiple health professionals and services. This will be achieved by developing new ways of delivering services and create stronger links between hospitals, paediatricians, paediatric nurses, GPs, allied health staff such as physiotherapists, and other agencies to ensure that care is coordinated, timely and appropriately. Consumers are involved in all Health for Kids activities. For more information click here We are looking for a group of parents and carers who are interested in giving their perspective to the activities within the project to help us meet the needs of the children and families who use these services. To assist you to take part in our project we will provide training and ongoing support, and cover any expenses associated with your participation. |
